Help me give my students a middle school survival pack in place of their moving up ceremony!

With the novel, COVID-19 pandemic hitting our schools, teachers are working to have students meet online in video chats and complete school work. This is no easy feat. There are so many barriers that can prevent a student from completing work including family, emotions, disabilities, and technology issues. BUT THIS HAS NOT HELD MY CLASS BACK!
The class has consistent effort and work completion from 12 out of my 14 students.
These students follow a schedule set for them, video chat, give feedback to one another, and ask questions. You wouldn't know it was their first time learning online or that they have language barriers and disabilities that already make learning a challenge. I could not be more proud of them.
Unfortunately, this pandemic has cancelled the rest of our school year including our moving up ceremony that allows us to celebrate that our fifth graders are off to middle school. They were the leaders of our school that volunteered in different classrooms and at different recesses to help end bullying and be role models for younger students. They will not be getting the send-off they deserve.
My students typically have a moving up ceremony and yearbooks to relive their time at our school. That will not happen this year, and I still want to wish them luck next year.
That's why I am making them a Middle School Survival Kit to send to their home!
I already have many of the traditional items, but these two items---the guide and the planner---are key components to finish off this kit for them. They will be able to use all of the organization skills we practiced this year and read some witty but helpful stories about middle school to ease this transition.
