Teach the Math and Science of Hockey

We’re excited to give you and your 4-7th grade students access to Future Goals Hockey Scholar, a series of digital lessons created by EVERFI and sponsored by the NHL and NHLPA. The Future Goals program teaches students standards-aligned STEM skills using interactive gameplay and real-world scenarios. Our mission is to help expose students to STEM concepts and skills and to peak curiosity and interest in the field.

In this survey, you'll be asked to share two screenshots of your account dashboard showing at least 10 student completions of one Future Goals science lesson and one Future Goals math lesson. Please have that ready before starting the survey. If everything checks out after you fill out this form, you'll be emailed your $125 DonorsChoose gift code within two weeks.

Teachers can only fill out each classroom rewards form once, and can earn a maximum of 1 gift code for each form. Teachers will only be able to redeem 1 gift code from each opportunity. Please note, you will only be able to submit this form once. Our team is unable to make any changes or edits to your responses. Once you have submitted, you will not be able to retake the survey, and our team won't be able to re-open it for you.

Per our Terms of Use, teachers are expected to submit Classroom Reward responses representative of their own classroom students. A violation of this expectation is subject to action and review by DonorsChoose and may result in terminating your use of the site.
